- For children under 16 years of age, a check ONLY for $134.85
made payable to U.S. Department of State along with

If you need to travel in less than 5 business days, you
must go in person to a Passport Agency with proof of travel. We
offer a courier service for those that are traveling from Friday
pm to Thursday of the following week. Application must be typed
using Online Wizard on the site www.travel.state.gov. At the end
Click "Create Form".This Form must have 2-D Barcode
on the upper left hand corner of first page.Forms without the
Barcode are unacceptable.
