Federal Passport Office Mail-In Application Requirements

• You need a passport that was issued to you within the last 15 years and you must have been 16 years or older when that passport was issued.

• If you qualify you may fill out the form online or download:
  
  
  • Fill out DS-82 form online  
  • Download DS-82 form

  Then, print and mail the completed application form according to the instructions provided.

• Or send us a self addressed, stamped envelope requesting Form DSP-82:
