GENERAL REQUIREMENTS
- All applicants must be at least 18 years of age.
- All commissions are for a statutory term of five years and must
be renewed during the period three months prior to the expiration
date.
- Once your application has been accepted for filing by the Secretary
of State, you will be sent a qualifying form and a revised identification
certificate. You must then take an oath of office before the New
Jersey county clerk of the county in which you reside if you are
a New Jersey resident, or the county in which you work as a nonresident,
within three months of the commission date. Failure to do so cancels
the appointment. The $25.00 fee is not refundable.
- A Notary Public must notify, in writing, the Department of Treasury
and the County where he or she qualified, of any change of name
or address before signing his or her name to any document which
he or she is authorized to sign as a Notary Public.
INSTRUCTIONS
- Please fill in all the information
on the form. Please indicate whether this is an initial application
or a renewal. The signature of the applicant should be the same
as used when notarizing documents. All applications and renewals
must be endorsed by a State Senator or Assemblyman after he or
she has examined the form. All applications and renewals, accompanied
by a check or money order for $25.00, payable to the State
Treasurer, should be mailed to the NOTARY PUBLIC SECTION,
DEPARTMENT OF STATE, P. O. BOX 452, TRENTON, NJ 08625. Facts
stated in the application or renewal are regarded as if made
under oath and a false statement will be cause for revocation
of the applicant's commission. Further, the applicant acknowledges
that he or she is familiar with the Notary law of this state
and, if appointed and commissioned as a Notary Public, will perform
faithfully, to the best of his or her ability, all notarial acts
in accordance with the law.
- Your application will be rejected
if all 11 boxes are not completed.
